Registered Parishioners
- If you are a registered parishioner and your child was baptized at Annunciation (or you have already given us a copy of their baptismal certificate), and if your child(ren) participated in faith formation in the 21-22 school year, all you have to complete is the information in the online "Parishioner Registration" form.
- If you are a registered parishioner and your child was NOT baptized at Annunciation, OR it has been more than a year since your family has attended faith formation in our program, please complete the online "Parishioner Registration" AND the PDF 2022-2023 Family Faith Formation Registration Form.
NON PARISHIONERS
- For all non-parishioners, please complete the online Non-Parishioner Registration and print out the PDF 2022-2023 Family Faith Formation Registration Form and scan it, then email it to [email protected], or you may mail any required documents such as baptismal certificate and previous faith formation records to:
